I don't think it's widely used, although without statistics or surveys it is difficult to say for sure.
Personally, I have been running a Misfin receiver at jk@skyjake.fi for little over a year and have been in contact with a total of six people using it.

TBH, I don't think so, OTOH: how often do you need to write message (think of email) to other geminauts?
I have few obvious ideas, that could make it more interesting, although it would likely turn it into something else...

To be brief, it is not an email replacement. I have no insights about the spam protection, as it allows generating a lot of .gmis files limited to 1MB. Additionally, I definitely want to see a GUI client, although I don't see much sense in creating one since there are no users to interact with.
Anyway, Misfin is something I'm really intrigued by as a minimalistic alternative to the giant privacy-less technologies, so I'm following the project news.

In ten months of running a misfin server I have yet to have ANYONE contact me over misfin.
(I did sign up on another server just to make sure my misfin is in fact working.)

I found it as notification service. I've added action on gemini memtions to send misfin to me and in computer I have widget to count new messages
