I am very curious about the practical aspects. What equipment do you use to connect and what baud rates? How do you negotiate the protocol/rate?

In the old days you'd use a TNC, terminal node controller connected to a radio. Nowadays a radio and soundcard are sufficient. Protocol is predetermined by what you're operating in (packet, BBS, Winlink, APRS) and BAUD is usually determined by what frequency you operate in. 300 for HF, 1200 for VHF, 9600 for UHF.
