The obsession with trans issues from both sides in the US is most unfortunate.

But humanitarian stuff is mostly bullshit - a way to pump money into NGOs and get cheap labor and resources.

As to what's really happening, your guess is as good as mine. The press is not used uo being around a president that thinks outloud -- last administration ignored the press except for propaganda.

mass media scammers ask for government money to call themselves 'independent'

If this is in reference to the White House's claim that USAID spent 8 million dollars to fund Politico, that claim was false. USAID paid them $44k in institutional subscription fees.

Several of the other claims the White House made about USAID spending were also false. Funny thing to lie about given the spending for US agencies, including USAID, is public at usaspending.gov

Trump when asked about USAID he will sometimes refer to the plight of white South Africans who are on the losing end of a land reform law. He wants to cut off aid to South Africa because of it. Why does he care? As an American I think about South Africa only slightly more than I think about the rest of Africa: hardly at all! Hey, wait a second! Isn't Elon Musk a white South African whose family stands to massively lose because of the land reform law?! The transgender musicals in South America is the culture war rage bait to distract from the true reason.

The presence of Musk definitely adds a foul odor to all things he is involved in.
